If you are bringing your pet to our clinic for the first time and have already scheduled an appointment, please complete this Fear Free Pre-Visit Questionnaire prior to your visit. Please note, this is not an appointment request form and any responses received that do not match a pet in our system will be discarded.

Our goal is to provide quality care to all our patients in a timely manner. No-shows and day-of cancellations not only inconvenience our team, but prevent us from providing care to other patients. For non-established clients, we require a deposit over the phone for the examination fee at the time of scheduling.
When you book your appointment, you are holding a space on our schedule that is no longer available to other patients. If cancellation is necessary, we require that you call at least the day before your appointment. Appointments are in high demand, and your advance notice will allow another patient access to that appointment time. If you need to cancel your appointment, please call us during business hours. If necessary, you may leave a detailed voicemail message and we will return your call as soon as possible. In the case of a no-show or day-of cancellation or late arrival of 10 minutes or more, we reserve the right to keep the exam deposit that was paid at the time of scheduling.
